Transitions signify change which can be a gift, and also can present challenges for our teams, and our students. Change accompanies a range of big emotions, including hope, because at least some uncertainty is certain. However, what we can control is our approach, ensuring we pause to thoughtfully plan with all of the information we have, consider the pros and cons of ways to move forward, and choose the path of transformative success. 
Transitions further provide us with a rich opportunity to reflect on:
  • what has been working well
  • what hasn’t been working
  • what has worked to mediate or overcome barriers
  • potential areas of focus to learn, grow, and move forward in a better way

Please refer to our Support for Learning website Transitions page for documents to support transition and see the documents listed below for transition support specific to our students moving from elementary to middle and from middle to secondary.

School teams are confident and familiar with supporting transitions for most students following typical transition timelines and processes. This document is intended to provide school teams with guiding considerations regarding transition planning for the few students having significant support needs.

Planning and Programming for Year 13 

Occasionally a student and their family will choose to attend school for an additional year following Grade 12, to support their pre-transition to adult life. In this case it is necessary for secondary school teams to consider how this additional year will
 be planned to ensure continued student growth and development that is relevant and tailored to the student’s anticipated transition to community services.

DESIGNATIONS SPRING REVIEW 2024

The week of April 8-12 school administration will receive the list of the specific students in their school who are identified to be reviewed. This Spring Designation Review specifically focuses on students with ASD, PDCH, IBI or MBS designations. 
When your school receives the list:
  1. Review the list of students who are identified for review with your School Based Team.
  2. All identified designation re-submissions should be uploaded to the Referral app by Friday, May 24, 2024.  Please follow the directions regarding what documentation is required for the review.

ONGOING DESIGNATIONS
Designation requests continue to be processed through the District Referral App. The Designation Review Committee  (ASD, IBI, PDCH)  meets the 3rd Wednesday of each month to review new designation requests. If you could use support collaborating to coordinate a designation package or in using the app, please connect with your DLST.

As we continue to partner with community based services and agencies, we wanted to remind teams of this document that outlines best practices when looking at community based services to support students within our schools.

CYMH CONSULTATION & COLLABORATION

We will be offering Spring Meetings with CYMH Clinicians for returning and new students to support access to shared planning with a community agency.  Indigenous CYMH, Victoria CYMH, and Saanich CYMH Clinicians will be available to meet with Case Managers via Microsoft Teams.
THE DEADLINE:  Friday, April 5, 2024  for all required documentation for new and returning students to be emailed to Tara Leach.

Once you have emailed all the required documents, you will receive a link to book your meeting(s) with the clinician.

CEREBRAL PALSY ASSOCIATION OF BC

The Cerebral Palsy Association of British Columbia (CPABC) is currently engaged in a Cerebral Palsy Awareness Project on Vancouver Island. They also have a variety of resources available for families, individuals, and schools.
CPABC was started in 1954 by a group of parents who wanted to assist their children living with cerebral palsy to reach their maximum potential within society. Today they are an independent charitable organization governed by a volunteer Board of Directors. They provide support, education, and information throughout BC.

LIVES IN THE BALANCE ANNUAL SUMMIT
ON COLLABORATIVE & PROACTIVE SOLUTIONS

The theme of this year's virtual Summit is "Voices of the Neurodivergent Community"
There will be wide array of breakout groups on topics related to neurodiversity and the Collaborative & Proactive Solutions (CPS) model. If you can't join in live, your free registration gives you access to the recordings for two weeks following the live event.
